Frequently Asked Questions

Coastal Integrative Psychiatry — Victoria Miller, PMHNP-BC

What is integrative psychiatry?

Integrative psychiatry blends evidence-based medication management with holistic therapies that address the mind, body, and nervous system. This approach goes beyond symptom relief by exploring root causes such as trauma, lifestyle factors, nutritional deficiencies, and emotional patterns. My goal is to create a personalized treatment plan that supports long-term healing, not just short-term fixes.

Do you take insurance?

I am an out-of-network provider, meaning I do not bill insurance directly; however, I provide a detailed superbill that you can submit to your insurance company for reimbursement of out-of-network benefits. Many clients are able to receive partial reimbursement depending on their plan.

What can I expect during the first appointment?

Your initial visit lasts at least 90 minutes and is intentionally thorough. We explore your history, symptoms, trauma background (if any), lifestyle patterns, past medication responses, and goals. This session helps me build a deep understanding of your story so your treatment plan is tailored, safe, and effective.

Do you offer therapy in addition to medication management?

Yes. I provide trauma-informed therapy, EMDR, ego-state/parts work, nervous-system stabilization, and emotional-regulation skills alongside medication management. Many clients appreciate that I can offer both therapy and integrative psychiatric care in one place. If long-term therapy is needed, I have a trusted network of therapists to whom I refer my clients.

Do you offer EMDR?

Yes. I am EMDRIA-trained and offer EMDR for trauma, chronic anxiety, attachment wounds, and emotional dysregulation. EMDR is always paced gently to ensure safety and stabilization before processing deeper material.

What is pharmacogenetic testing (GeneSight®️)?

Pharmacogenetic testing is a cheek-swab test that analyzes how your body may metabolize certain medications. I offer GeneSight®️ testing to help reduce trial-and-error and guide more precise medication choices. This test is optional, but can be highly informative for people who have had multiple medication trials with limited success.

Do you recommend supplements or nutraceuticals?

I offer medical-grade, professional-grade nutraceutical guidance based on your symptoms, lab work, and overall health. I use evidence-based, medical-grade neutraceuticals—not over-the-counter “trendy” products—to support mood, sleep, inflammation, and nervous system regulation.

Do you provide telehealth appointments?

Yes, I offer secure, telehealth sessions to adults across South Carolina. Many clients choose a blend of in-person and virtual visits based on their schedule and comfort.

Can you help if I’ve tried many medications without success?

Absolutely! Many of my clients come to me after multiple medication trials. My integrative approach looks beyond medications alone, exploring trauma history, attachment patterns, nervous system regulation, labs, hormonal influences, nutrition, sleep, stress, and lifestyle. Together we work to understand why past treatments haven’t worked.

What conditions do you treat?

I specialize in:

  • Chronic anxiety
  • Depression
  • Trauma & PTSD
  • Attachment wounds
  • Emotional dysregulation
  • Chronic stress & burnout
  • Life transitions
  • Chronic pain and fatigue
  • Women’s mental health
  • Medication-resistant symptoms

If you’re unsure whether I can help, I offer a complimentary consultation to discuss your needs.

How often will I need to be seen?

Frequency is individualized. Typically:

  • Initial appointment: 90 minutes
  • Follow-ups: every 4–6 weeks (or sooner if needed)
  • EMDR or therapy sessions: weekly or bi-weekly

I never rush sessions, and everything is paced to your comfort level. You don’t need to change therapists to work with me. I often collaborate with outside therapists to coordinate care, support medication management, and create an integrated treatment plan tailored to your needs.

What age groups do you work with?

I work with adults (18+), with a special focus on women navigating life transitions, anxiety, stress, or trauma-related symptoms.

What makes your practice different?

Clients often say they feel truly seen and heard for the first time. I take time to understand your story, explore root causes, and use a combination of medication, therapy, and integrative tools, not a “medication-only” model. My practice prioritizes compassion, education, safety, and long-term healing.

What if I’m nervous about starting medication?

It’s very common to feel nervous about starting psychiatric medication, and your concerns are always respected at Coastal Integrative Psychiatry. I practice integrative, trauma-informed, medication management, which means decisions are made collaboratively and never rushed. We take time to review the potential benefits, risks, and alternatives, and medication is always offered as one option within a holistic mental health approach, not a requirement.

When appropriate, I partner with trusted, local, compounding pharmacies, allowing many clients to begin with lower-dose or customized medications. This gentle, gradual approach helps reduce side effects and supports nervous system safety. Most importantly, we move at a pace that feels right for you. My goal is for you to feel informed, supported, and in control of your care every step of the way.
